Student Absence Request Form

This form must be turned into the office two (2) academic days prior to an absence.

Excused absences will only be granted for medical appointments, family emergencies, milestone Family events, honor study days, illness, religious observances, college visits, or official school activities. Please do not request an Excused Absence unless it meets one of these criteria.  This form must be turned into the office at least two (2) academic days prior to the day(s) of absence or the absence may be noted as unexcused. Not doing so may result in detentions.
