Welcome!  We hope the following form will help streamline the book ordering process so that materials will arrive in the library in a timely manner.  Please use the following form to request books that your classes would use in the library, as well as items that you think would benefit the library here at Cary Academy. Please read the Acquisitions Policy if you have any questions concerning appropriateness of material.  The library will try to let you know within 3 business days if we can order the item requested  If an item cannot be ordered, you will be informed of this decision by the librarian. 

You do not need to know all of the information asked for below. Please just fill in what you do know about the book.  Don't forget to check the On-Line Catalog to see if we have the book you want.
