Physical Space Questionnaire

Please feel free to attach additional sheets, including detailed descriptions of suggested new programs, details of space parameters for such programs, (size, location) and any other information that you may have regarding your suggestions. You may certainly address as few or as many of the questions as you would like. Please use the enclosed envelope to return forms to Carol Winslett by February 28, 2000.
