Fentanyl is created in a lab. Some fentanyl is created in illegal labs and used illegally on the streets, however much of fentanyl is created in medical labs and used for medical purposes. In its natural, pure form fentanyl is highly toxic and if used can be fatal. So as far as its natural abundance it is very low because almost immediately after its creation it is diluted to reduce toxicity.
