T-Shirt Order Form

We will order the T-shirts next week.  Please send in this form and a check made payable to CA for the T-shirt - $8.00 per shirt.  Also, please put your check in an envelope with your child’s name on the outside  - stating it is T-shirt money.  Please send in by Tuesday, April 29.

VERY IMPORTANT – the Chick-Fil-A and T-shirt money should be separate.  Different parents are in charge of coordinating the shirts and the dinner.

Do remember these shirts are a costume and buy accordingly.  They will need to be tucked in so buy to look good – not to sleep in.  J  There is no children’s XLarge because they are the same as Adult Small.
